Aims : to discuss recommendations for management of the nonexacerbating patient with severe COPD within the domains of identification and reduction of risk factors, pharmacologic and nonpharmacologic treatment modalities, and monitoring and follow-up after having elucidated the evidence behind these recommendations; to present the possibility of treating patients with COPD using a step-up and also a step-down approach; to emphasize the important role of evaluating and managing comorbidities is crucial in patients with severe COPD (mainly for cardiovascular diseases); to discuss the issue of polypharmacy in patients with severe COPD.
